In het praktijkpand is het niet toegestaan om te roken<\/h4>\n\u25cf We vragen u om uw mobiele telefoon uit te zetten tijdens de behandeling.<\/h4>\n\u25cf Het is de verantwoording om van de cli\u00ebnt om te controleren in hoeverre uw zorgverzekering de fysiotherapeutische behandelingen vergoedt. Er wordt dan ook aangeraden om dit van te voren te controleren zodat u niet voor verassingen komt te staan.<\/h4>\n\u25cf In de wet gebruik burgerservicenummer in de zorg, is vastgelegd dat de zorgaanbieders het bsn-nummer moeten gebruiken in hun administratie en dit moeten controleren. Daarom \u00a0wordt er bij de eerste afspraak gevraagd om een geldig legitimatiebewijs en de zorgpas.<\/h4>\n\u25cf Fysio Toet is niet aansprakelijk voor verlies, diefstal of beschadiging van eigendommen van de cli\u00ebnten in of rondom onze praktijkruimte<\/h4>\n\u25cf Fysio Toet streeft ernaar om de behandeling op het geplande tijdstip te starten. Mocht de behandeling meer dan 10 minuten afwijken van het geplande tijdstip wordt u hier z.s.m. van op \u00a0\u00a0\u00a0de hoogte gesteld.<\/h4>\n\u25cf Mocht u door een behandeling gebruik maken van de fitnessruimte dan verzoeken we u vriendelijk om gebruik te maken van binnenschoenen en gepaste (sport)kleding.<\/h4>\n\u25cf Wij verzoeken u op de afgesproken tijd van de behandeling aanwezig te zijn. Mocht u verhinderd zijn dan dient u dit tenminste 24 uur voor de afspraak te melden. Gebeurt dit niet dan zijn wij genoodzaakt om de behandeling in rekening te brengen<\/h4>\n\u25cf Vraag de therapeut uw volgende afspraak op het gegeven afsprakenkaartje te noteren. Dit voorkomt misverstanden en onnodige telefoontjes die de\u00a0behandelingen onderbreken.<\/h4>\n\u25cf Fysio Toet aanvaart geen enkele aansprakelijkheid voor persoonlijk letsel van cli\u00ebnten, welke zich niet aan de instructies van de fysiotherapeut hebben gehouden.<\/h4>\nPrivacyreglement<\/h4>\n<\/h4>\n\u25cf Fysio Toet houdt, om uw behandeling zo goed mogelijk uit te kunnen voeren, een registratie bij van uw medische en administratieve gegevens. Op deze medische en administratieve registratie is de Wet op de Persoonsgegevens van toepassing. Deze wet bevat regels met betrekking tot het doel \u00a0\u00a0\u00a0van de registratie, de aard van de gegevens die worden geregistreerd, het beheer van de gegevens, de personen die toegang hebben tot die gegevens en het inzagerecht van de pati\u00ebnt. Al deze zaken zijn in een reglement vastgelegd.<\/h4>\n\u25cf Uiteraard gaat uw fysiotherapeut zorgvuldig met de gegevens om: daarom is naast de bovengenoemde Wet op Persoonsregistraties, een aantal regels vastgelegd in een \u00a0\u00a0\u00a0privacyreglement. Onderstaand is het reglement weergegeven:<\/h4>\n\u25cf Behalve de fysiotherapeut die u behandelt heeft ook een beperkt aantal andere personen toegang tot de gegevens die in de registratie zijn opgenomen. Dit zijn bijvoorbeeld de waarnemers, andere fysiotherapeuten in de praktijk, eventuele stagiaires en administratieve krachten. Al deze personen hebben een geheimhoudingsplicht.<\/h4>\n\u25cf Omdat Fysio Toet kwaliteit van verslaglegging periodiek door een externe instantie laten controleren, zou uw dossier hierbij ook ingezien kunnen worden. Hiervoor vragen wij apart uw toestemming.<\/h4>\n\u25cf U heeft het recht om de gegevens die over u zijn vastgelegd in te zien. Indien u meent dat gegevens onjuist zijn vastgelegd kunt u de behandelende fysiotherapeut verzoeken deze te wijzigen. Alleen gegevens die te maken hebben met de behandeling worden in het medisch dossier opgeslagen.<\/h4>\n\u25cf Gegevens van cli\u00ebnten worden niet aan derden verstrekt ook niet aan naaste familie. Gegevens worden pas vrijgegeven nadat de cli\u00ebnt hiervoor toestemming heeft gegeven.
